Coram continuously captures a comprehensive array of data points for each patient referred to us. These data elements are gathered from admission through discharge, and are documented by various members of Coram’s multi-disciplinary service team.
Coram’s proprietary outcomes database is used internally and is shared with payors, physicians, and hospitals to provide feedback on various utilization, pharmacoeconomics, patient satisfaction, patient adherence, and therapeutic outcomes.
To measure the cost-effectiveness of our therapy management approach, we
use our
outcome reporting system to monitor cost-avoidance and/or reductions
from product
utilization, reduced hospital stays, failure
to respond to therapy,
emergency room
visits, adverse reactions, catheter complications,
and
a
variety of
other measures of
clinical and quality outcomes.
Patient Satisfaction Surveys
Our Customer Satisfaction Survey process measures patient, caregiver,
physician,
referral source, and case manager expectations of home
infusion services against
their
actual experience. Coram continuously
surveys new, existing, and
discharged
patients
throughout their course
of treatment.
Coram also surveys select referring physicians and periodically surveys
discharge
planners. Survey responses are reviewed and results provided
as
individual
branch
and company-wide reports. Improvement
opportunities
are
addressed
as
needed.
Employing the services of an
independent,
third-party
expert
healthcare
industry
survey firm,
patients are surveyed
via mail at periodic
service intervals
and at the time
of discharge.
